Patient & Family Information

Out-patients:

To schedule or change an appointment, call (+250) ………. (free call, MTN) or visit our hospital website and request an appointment on www.chuk.rw.
Every new out-patient has to request a patient ID number in order to allow him to process all his clinical procedures.

The patient can also visit the information and appointments desk at the CHUK hospital to request an appointment in person or send his relative.

All out-patients must have got their scheduled appointment. Emergency clients are not required to make online appointment time; they are redirected straight to ED after a call from referring hospital.

In-patients:

Upon arrival, go to the admissions desk to fill out an admissions form. The admissions officer will guide you to the ward with the help of a porter, if needed.
You must pay your deposit fees according to the type of the ward prior to your procedure. Once the procedure has completed, your doctor will visit regarding recovery follow up and information.

While in recovery, a billing will be made and a billing clerk will discuss your bill with you so you understand the charges.

The doctor will inform you when you will be discharged. You will receive you bill after your discharge and will pay the bill in the cashier desk.

Emergency:

If It is an emergency, please stop reading and call the hospital immediately.
Upon entrance to the Emergency Department, you will be directed in the triage room.

The nurse will carry out a patient assessment and determine one of three levels for the patient’s condition, which determines where the patient is taken.

Patient will see a doctor after nurse’s reception and assessment. The patient registration will be completed by their caretaker. Otherwise, the patient will complete the registration after he or she has been treated.

Patients when they enter the emergency room are seen by the doctor, his or her diagnosis will be entered into the HMIS called Openclinic and all subsequent tests are automatically sent to the lab, pharmacy, etc.

All patients will pay at the end of their visit to the emergency department to the Cashier, once they are discharged, and they will pay in the wards the deposit fees if they are transferred to the wards at the end of their stay.

b. While in the Hospital

Arrival

When you arrive, visit the Outpatient Department Reception area (OPD polyclinic, Clinic, Dermatology, etc). If you are a new patient, you will be asked to complete new-patient identification and get a patient ID. If you have health insurance and transfer note, please bring proof of them with you. If you are an existing patient, please let the receptionist know of any changes to your profile, check the transfer note, your patient ID and reason for encounter.

Billing

If you have any outstanding balances, you will be asked to visit the cashier to clear it. If you cannot pay in full, you will be directed to credit control and to come back when your charges are cleared. You will pay a consultation fee to the cashier clerk before your medical visit.

Appointment

You will be directed to the triage room for a patient assessment. After this assessment the triage nurse will direct you to the doctor’s room, where the doctor will see you within 15 minutes.

After you are seen by the doctor, you will be directed to the lab or pharmacy. The doctor will have electronically sent request to the lab, and they will prepare your tests or medications before or soon after you arrive.

When you enter the pharmacy, medical imaging or lab, you will receive service and pay for them at those locations. If a lab test is given, your results are entered into the system and your doctor will check them. You will return to the doctor’s room to discuss your results with your doctor. Before you leave, you will visit the pharmacy to pick up your prescribed medications.

c. Back at Home

While at home, it is important to follow your doctor’s recovery instructions. If you feel that your recovery is not going normal, it is your responsibility and right to contact your doctor and discuss the issue.
